Insurance/Financial Information

We are providers of several different insurances and will file any and all insurance as a service to all Practice Members. Individual and family payment packages are available to help fit your budget.

Private Insurance/Medicare

Patient is responsible for deductible, co-pays and all denied or unpaid charges according to your contracted policy. Your coverage is a contracted policy between you and your chosen company. Lifestyle Chiropractic Center (LCC) is just a third party.

Patient Pay

Patient is responsible for payments at time of service at rate of $45/visit and all other charges associated with the time of service. ie: x-rays, support devices, therapies, etc.

Pre-Pay Patient Packages

Pre-pay discounts are available for patient charges for a predetermined number of visits or services.

Financial Hardship Situations

To be discussed with Office Manager and or Doctor.
